have the right to be treated with courtesy, compassion, and
respect by the doctors and staff of this dental treatment facility.
Your reciprocity of this kindness is appreciated.
You
have the right to receive information regarding your dental
diagnosis, treatment, treatment alternatives, expected outcomes,
associated risks, and estimated costs to assure your informed
choice. It is your responsibility to proceed with treatment
as recommended; seek a second opinion, or refuse treatment.
You
have the right to confidential treatment of your dental and
financial records, and may approve or refuse their release,
except when required by law.
You
have the right to change dentists, or any treatment co-therapists,
is you so choose.
You
have the right to express grievances or make suggestions by
addressing our patient contact representative.
