Patient
time spent with your doctor (dentist) is very important to us
and appointments are scheduled to provide ample time for patient
consultation.
If you cannot
keep an appointment, please notify us immediately. We ask that
you give us 48 hours notice. This courtesy makes it possible
to offer your appointment time to another patient who desires
to see the dentist or hygienist.
It is requested
that payment be made at the time services are rendered on the
first visit to our facility. After development and discussion
of a dental treatment plan for you, several financial payment
options are available to assist you.
For those
patients who have dental insurance, please provide a fully completed
form at your visits. We will be happy to send these forms to
your insurance carrier as a courtesy to you. The Dental Arts
Center accepts MetLife, Delta Dental, Guardian and Aetna/Assurant
dental insurance. We’ll gladly file claims with other insurance
providers as a service to you. We will do our utmost to help
you receive maximum benefit; however, the agreement of the insurance
company to pay for dental care is a contract between you and
the company. Special discounts are available for prepayment
of fees over a certain amount and for senior citizens over age
65. Specific information is available at our office.
For your
convenience, cash, checks, Visa, Discover, MasterCard and American
Express are accepted for payment.
